The purpose of this study was to evaluate the difference in the refractive errors and the intraocular lens (IOL) powers by comparing predicted refractive error and postoperative spherical equivalent after sulcus implantation of IOL. The reoperative expected refractive error, postoperative refraction, actual IOL power and virtual (in the bag) IOL power for the same ostoperative refraction in 20 eyes with normal axial length after sulcus fixation were retrospectively evaluated. A myopic shift (mean -1.09+/-0.72D) from the predicted refraction was developed (p<0.05). After sulcus implantation, the difference between actual IOL power and virtual (in the bag) IOL power was 1.18+/-0.83D (p<0.05). The IOL power for sulcus implantation should be 1.1D less than the power for in the bag fixation.
